Evaluation/Consultation

A comprehensive assessment is essential to determine what is best and what is feasible, so that necessary plans can be put into action. SSC will prepare a written report for you that will summarize the following:

• safety concerns at home
• physical, cognitive and functional limits
• recommended level of care (setting)
• cost estimates for care options
• insurance coverage and gaps
• family assets and asset protection
• legal documents (health care proxy, will, power of attorney)
• consultation with physicians, lawyers, financial planners, etc.
• referrals to area specialists and community resources

Care Selection

Although the ultimate decision must rest with the family, SSC will accompany your when choosing a care facility. We will help you to:

• tour and interview nursing homes or assisted living centers
• determine which facility is best equipped to meet specific needs
• choose the best home care providers
• interpret the results of state surveys
• evaluate how a facility measures up against its peers

Care Management/Advocacy

SSC will be a resource as long as we may be of service, knowing that making decisions about a loved one's care is not an event but a process. We offer:

• continued on-site visits
• attendance at care plan meetings
• service as liaison for out-of-town family members
• ongoing evaluation of senior's changing needs
